We are open Monday to Friday, from 7AM-4PM & Saturday from 7am-12pm. During these hours, our team is readily available to assist you with any inquiries, requests, or services you may require. Extended Hours and Extra Fee: For clients who require our services before or after our regular business hours, we offer extended availability to accommodate your needs. However, please note that any services provided outside of our standard business hours will incur an additional fee.
To schedule an appointment, you have the convenience of choosing from various methods such as online, phone, text, or email. Once you have made the request, our dedicated team member will promptly reach out to you to confirm the appointment and provide any essential instructions. When scheduling, kindly provide the following details: the patient's full name, date of birth, phone number, email (optional), and the specific services you are requesting.
Before the appointment, it is necessary to send us the requisition form either by fax or email. This can be done by either the healthcare provider or the patient themselves. It is important that the requisition form is signed by the ordering provider. Once received, I will thoroughly review the requisition and notify the patient if any additional information is required.
